YOGA for the Special Child (& Siblings!)
presented by Creating Choices for Children of C/WC
Join CCC and instructor Jennifer Fowles of Kiddie Yoga in Verona for some stretching
and relaxing!
When: Saturdays, Sept. 6, 13, 20 & 27 and Oct. 4, 11, 18 & 25, 2008
Time: 11:30am - 12:00pm (ages 3-8)
Where: Presbyterian Church of West Caldwell, 31 Fairfield Avenue,
West Caldwell, 07006
Cost: $65 per child for 8 week series ($75, if after 8/25)

Insurance Waiver & Release of Liability
In consideration of being allowed to participate in any way in programs and activities sponsored by Creating Choices for Children of Caldwell/West Caldwell (CCC), I and/or the minor participant, for myself, the minor participant, and on behalf of our respective families &/or personal representatives, the undersigned:
- I will remain on the premises for the duration of the program in which the child participant is attending;
- I certify the child participant is healthy or suitably physically fit, or is able to fully participate under the parameters of the current program;
- I am aware CCC carries no liability insurance and my child participant’s involvement in this and all CCC programs is voluntary, and I cannot hold CCC or the facility hosting the event or program responsible if injury should occur;
- I understand that I accept personal responsibility at all times for my child participant in this and all CCC programs;
- I understand that at this program/event or related activities, I and/or the minor participant may be photographed and agree to allow said photo, video or film likeness to be used for any legitimate purpose by the program, for publicity or advertisement.
I HAVE READ THE ABOVE WAIVER & RELEASE, AND AGREE TO ABIDE BY ITS TERMS.
